Job summary

Custom Care Hospice is seeking an experienced registered nurse to plan, organize and direct hospice care in patients' homes and communities. The nurse will build from community resources to meet individual and family needs within a home setting.

The role includes initiating and coordinating plan of care, performing assessments, administering medications, counseling patients and families, and acting as a case manager to ensure continuous, compassionate care across the care continuum.

Qualifications

• Licensed registered nurse with current Texas license.
• Experience in hospice or community health care.
• Ability to plan and direct patient care.

Responsibilities

• Assess patient and family to determine hospice needs.
• Develop and revise care plans with nursing diagnosis.
• Administer medications as prescribed and educate family.
• Coordinate with interdisciplinary team and on-call duties.
