Job summary

Cadre Family Companies is seeking a dedicated Hospice Registered Nurse to provide compassionate, high-quality care to patients in home health settings, assisted living communities, and skilled nursing facilities. Responsibilities

Provide bedside care to patients and families.
Deliver care in patients' homes, assisted living communities, and skilled facilities.
Manage patient symptoms with clinical expertise and empathy.
Support patients and families throughout the hospice journey.
Educate on disease progression, end-of-life care, and self-care.
Help families understand their care and expectations.
Coordinate with the interdisciplinary care team for comprehensive care.
Document assessments, progress notes, and care plans accurately.
