LifeSource Hospice is seeking a compassionate Full-Time Hospice On-Call Registered Nurse (RN) to provide after-hours clinical support across Bergen and Passaic Counties, New Jersey. The On-Call RN serves as the primary clinical resource, responding to patient and caregiver needs through telephone triage and in-person visits as necessary.

Qualifications

• Current and unrestricted RN license in New Jersey.
• Minimum of three years of nursing experience.
• Minimum of two years of hospice nursing experience.
• Previous hospice triage and after-hours on-call experience.
• Ability to travel throughout Bergen and Passaic Counties.
• Excellent assessment, critical thinking, communication, and organizational skills.
• Valid driver's license and reliable transportation.

Responsabilités

• Serve as the primary after-hours clinical contact for hospice patients and caregivers.
• Respond promptly to incoming calls and perform telephone triage.
• Determine the need for nursing intervention and provide appropriate guidance.
• Conduct urgent and emergent nursing visits throughout Bergen and Passaic Counties.
• Assess patient condition and implement nursing interventions per physician orders and the hospice plan of care.
• Provide expert symptom management for pain, dyspnea, anxiety, and other end-of-life symptoms.
• Complete after-hours hospice admissions and conduct death visits as required.
• Coordinate care with hospice physicians, pharmacies, and multidisciplinary team.
• Maintain accurate EMR documentation and ensure care coordination.
