Job summary

LifeSource Hospice is seeking a compassionate RN to provide after-hours hospice support and evening tuck-in visits across the service area. This full-time role focuses on symptom management, patient comfort, and family education during overnight shifts.

The ideal candidate will have at least 5 years of nursing experience, NJ RN licensure, and the ability to work independently in the field. Evening/night shifts include on-call responsibilities with rotating holidays and weekends.

Qualifications

• Current RN license in New Jersey.
• Minimum of 5 years nursing experience.
• Hospice, home health, oncology, med-surg, or critical care experience preferred.
• Ability to work independently in the field.

Responsibilities

• Respond to hospice patient calls and urgent nursing needs after hours.
• Perform tuck-in visits to assess comfort, safety, and caregiver needs.
• Provide skilled nursing assessments and symptom management.
• Educate patients and families on disease progression and end-of-life care.
• Collaborate with interdisciplinary hospice team to ensure continuity of care.
• Complete timely EMR documentation per standards.
• Communicate changes in patient condition to physicians and leadership.
• Assist with crisis management, pronouncements, and admissions when needed.
