Job summary

3HC Home Health & Hospice Care Inc. seeks an Occupational Therapist to enhance patient independence through prescribed therapies. You will assess patients, develop care plans, and implement treatments under physician direction, while adhering to 3HC policies and reimbursement guidelines.

Responsibilities include supervising staff, documenting progress, and collaborating with the care team to achieve outcomes. Requires NC OT license and CPR certification, with prior clinical experience preferred.

Qualifications

• Graduate of an approved Occupational Therapy curriculum.
• One-year of clinical experience preferred.
• Licensed to practice occupational therapy in North Carolina.
• Maintain current CPR certification (BCLS).

Responsibilities

• Abides by and supports 3HC's Compliance Program and Code of Ethics.
• Creates positive experiences for internal and external customers.
• Manages assigned cases and assists office with patient outcomes and plan of care.
• Performs comprehensive assessments and develops plans of care with physician input.
• Provides supervision to Certified Occupational Therapy Assistant and CNA as appropriate.
• Maintains clinical and administrative records promptly and accurately.
• Maintains regular communication with supervisor and coordinates referrals and services.
