Compassus is seeking an Occupational Therapist to conduct therapy evaluations, develop and implement treatment plans, and discharge planning for patients in home hospice settings. The role collaborates with a multidisciplinary team to optimize function and safety in patients’ homes.

The position requires a California OT license, BLS, and at least 1 year of acute care, rehab, or home care experience. Strong communication, organization, and advocacy for patient goals are essential.

Formación

• Bachelor’s or Master’s Degree in Occupational Therapy required.
• Ph.D. in Occupational Therapy preferred (also acceptable in lieu of Bachelor’s/Master’s).
• CA: Coursework/Training in Treatment of Adult Hemiplegia and Hand Therapy preferred.
• 1 year experience in acute care, rehabilitation, or home care required.

Responsabilidades

• Evaluate patient function and safety to determine home-care needs.
• Develop and communicate plan of care with the hospice team.
• Educate patients and families on treatments and safety.
• Teach home exercise programs and adaptive techniques.
• Provide case management and coordinate services.
• Document assessments and communications per standards.
• Coordinate information with IDG and external sources.
• Support discharge planning and caregiver training.
• Obtain adaptive equipment as needed and ensure home safety.
