Job summary

Dignity Health at Home is seeking an Occupational Therapist to deliver skilled home health services. The OT will assess patients, set goals, and develop plans to improve independence and quality of life in home settings.

Requirements include a bachelor’s degree in OT, an active OT license in the practice state, and CPR/BLS certification. A driver’s license is required, and at least one year of OT experience is preferred.

Qualifications

• Bachelor’s degree from an accredited program approved by the American Occupational Therapy Association (AOTA).
• Active OT license in the state of practice.
• CPR/BLS certification current.
• One year of occupational therapy experience in a clinical setting (preferred).
• Valid driver’s license.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized plans.
• Teach patients and families about home safety, adaptive equipment, and strategies for daily activities.
• Maintain accurate records of progress, treatment plans, and changes to therapy programs.
• Monitor progress and adjust plans to meet changing needs using outcome-based measures.
• Collaborate with physicians, nurses, and other therapists to coordinate care.
