Qualifications

• Minimum 1 year experience required.
• Experience in home care, working with terminally ill and/or geriatrics preferred.

Responsibilities

• Delivery of patient care with personal assistance for ADLs.
• Attend interdisciplinary team conferences (IDT) and care conferences.
• Ensure accurate, complete clinical documentation in accordance with guidelines.
• Maintain clean patient rooms and proper stocking of supplies; ensure proper chemical use.
• Maintain licensure and CPR certification; ongoing professional development.
