Job summary

VIA Health Partners is seeking a compassionate Nursing Assistant to deliver in-home patient care, including bathing, mobility, nutrition support, and safety precautions in accordance with care plans.

The role requires listening to patient needs, documenting care, and collaborating with the interdisciplinary team to ensure high-quality end-of-life and palliative care. Prior home care or geriatrics experience is preferred.

Qualifications

• Minimum 1 year experience in home care or geriatrics preferred.
• Experience with terminally ill patients is a plus.

Responsibilities

• Deliver personal care (ADL) to patients including bathing, transfer, and nutrition support.
• Assist with bowel/bladder management and wound care as directed.
• Maintain safety with infection control, documentation, and adherence to care plans.
• Participate in interdisciplinary team conferences and communicate patient needs.
