Job summary

The Pennant Group is seeking a hospice healthcare professional in Idaho to provide medical care, palliation, and case management for terminally ill patients under physician supervision. This role includes performing histories and exams, developing care plans, and coordinating hospice services.

Responsibilities include conducting hospice face-to-face visits, contributing to eligibility decisions, and collaborating with the interdisciplinary team to ensure symptom control.

Responsibilities

• Provides medical care, palliation, and manages terminal illness under supervision of a collaborating physician.
• Completes history and physical examinations and documents findings in the medical record.
• Develops individualized plan of care, prescribes medications or treatments, coordinates hospice care services.
• Completes hospice face-to-face visits to assess eligibility for subsequent benefit periods.
• Provides clinical information to the hospice medical director for eligibility decisions.
• Makes recommendations to the hospice medical director regarding certification and recertification for hospice care.
• Reviews clinical information and coordinates patient care with the interdisciplinary team.
• Ensures patient receives appropriate symptom control through team collaboration.
• Reviews protocols and proposes intervention options in collaboration with the collaborating physician.
