Job summary

St. Croix Hospice is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision.

You will assess patients, develop plans of care, prescribe medications, and coordinate with the interdisciplinary team to ensure comfort and dignity for patients and families. You will conduct face-to-face assessments, manage symptoms, guide care plans, and collaborate with physicians, nurses, social workers, and chaplains to support end-of-life care in home settings across

Qualifications

• Must have current NP license in state of practice.
• Graduate of an accredited NP program with clinical training.
• Minimum two years RN experience; hospice or palliative care preferred.

Responsibilities

• Provide medical care and symptom management for terminally ill patients.
• Develop individualized care plans and coordinate referrals.
• Complete face-to-face assessments for hospice eligibility as needed.
• Collaborate with interdisciplinary team and physicians to adjust plans of care.
• Maintain accurate medical documentation and HIPAA compliance.
