You will evaluate patients, develop and direct plans of care, prescribe medications, and coordinate with families and the care team during home visits. The role emphasizes symptom management, collaboration with the interdisciplinary team, and adherence to regulations, HIPAA, and quality initiatives.

Qualifications

• Must have current NP license in state(s) of practice.
• Degree from an accredited school of nursing and Nurse Practitioner program.
• Minimum two years RN experience.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License and access to a reliable automobile.

Responsibilities

• Provides medical care and symptom management for hospice patients under physician supervision.
• Performs comprehensive history and physicals, orders/tests, documents findings.
• Develops plans of care, prescribes meds/treatments, coordinates referrals.
• Completes face-to-face assessments for hospice eligibility and reports to medical director.
• Ensures symptom control and collaborates with the interdisciplinary team.
• Educates patients and families on end-of-life care and supports continuity of care.
