Job summary
St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to deliver in-home, 1:1 holistic care to patients and families.
You will collaborate with an interdisciplinary team to create individualized care plans, guide end-of-life conversations, and ensure comfort and dignity in the home setting. You will provide nursing care, education for patients and caregivers, and participate in on-call rotations to support seamless transitions in care.
Qualifications
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience required; hospice/home health experience preferred
- Must be able to perform comprehensive assessments and develop care plans
- Educate patients and caregivers on care techniques and symptom management
Responsibilities
- Perform initial and ongoing comprehensive assessments to determine hospice needs
- Develop individualized care plans with patient and family; update as needs evolve
- Administer medications and treatments as prescribed
- Educate patients and caregivers on care techniques, medications, diet, and symptom management
- Rotate on-call duties and ensure seamless care transitions
