Qualifications

• Graduate of an accredited nursing program.
• RN license in good standing to practice.
• 1 year of professional nursing experience within last 3 years or 2 years with BSN in public health/home care/hospice preferred.
• CPR certification and valid driver’s license required.
• Excellent observation, communication and problem-solving skills.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments.
• Provide professional nursing care using nursing process.
• Develop and revise the plan of care with patient and family.
• Initiate and adjust the plan of care as patient needs change.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and report to physicians.
• Educate patient and family on self-care and care plans.
• Ensure safe environment and continuity of care.
• Participate in interdisciplinary team meetings and on-call duties.
