Luminary Hospice is hiring a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. You will build upon community resources to deliver personalized nursing care, pain management, and emotional support for patients and families.

The role emphasizes assessment, care planning, medication administration, and coordination with the interdisciplinary team to ensure high-quality end-of-life care. Requires nursing license and CPR certification.

Qualifications

• Graduate of an accredited school of nursing.
• Preferred 1–2 years of recent acute care in an institutional setting, and 1–2 years in home care.
• Excellent observation, communication, and nursing skills.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and initiates a care plan with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on meeting nursing and related needs.
• Maintains and updates clinical notes and communicates with physicians as needed.
• Coordinates patient care with the hospice interdisciplinary team and community resources.
• Participates in on-call duties and ensures equipment and services are arranged.
• Fulfills ongoing professional development and quality improvement activities.
