Job summary

Cleveland Clinic Akron General Justin T. Rogers Hospice Care Center is seeking an Advanced Practice Provider (NP) to join the Hospice team on a part-time basis.

The APRN, licensed as a Nurse Practitioner, will collaborate with physicians, nurses, social workers, chaplains and home health staff to manage pain, symptoms and quality of life for terminally ill patients at the Akron facility. Shifts are 9:00 am–2:30 pm, 24 hours per week, with every 4th weekend coverage and on-call once per week from

Qualifications

• MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
• Graduate of an accredited APRN program.
• If in neonatal care service area Master's Degree is required upon hire.

Responsibilities

• Conducts thorough medical histories, performs complete physical examinations, initiates appropriate tests, and develops a working diagnosis and treatment plan.
• Works in collaboration with the health care team.
• Performs and interprets routine diagnostic procedures as credentialed and privileged by hospital committees.
• May prescribe and monitor medications as allowed by license through state boards and Cleveland Clinic policies.
• Educates patients about their disease, treatments, related drug effects and hazards.
• Participates in medical team rounds; collaborates with nursing and other healthcare team members.
• Refers patients to specialists as appropriate for consultation or for specialized resources.
• Ensures continuity of care by serving as a liaison with other care team members.
