Job summary

Cleveland Clinic’s Cancer Institute is seeking an Advance Practice Provider (APRN) to join the Hospice team at the Justin T. Rogers Hospice Care Center in Akron, Ohio.

The ideal candidate holds an MSN, has nurse practitioner credentials, and experience in hospice or palliative medicine. You will collaborate with physicians, nurses, social workers, chaplains, home health aides, and volunteers to manage pain, quality of life, and emotional needs.

Qualifications

• MSN required upon hire or within 3 years for external hires and internal moves to APRN license.
• Graduate of an accredited APRN program.
• If in neonatal care service area Master’s Degree is required upon hire.

Responsibilities

• Conducts comprehensive histories and physical exams and develops a treatment plan in collaboration with the multidisciplinary team.
• Performs routine and specialized procedures as credentialed by hospital authorities.
• Prescribes and monitors medications as allowed by license and Cleveland Clinic policies.
• Educates patients and families about disease, treatments, and potential side effects.
• Participates in rounds and coordinates care with nursing and other health care team members.
• Ensures continuity of care and appropriate referrals to specialists as needed.
• Contributes to quality monitoring and discharge planning to ensure regulatory compliance.
• Supports professional development of staff and translates evidence-based practices.
