Job summary

Haven Home Health and Hospice is seeking a Hospice Licensed Practical Nurse (LPN) to provide bedside nursing care to patients in whose home setting under the supervision of an RN. Travel within the service area and adherence to physician plans of care are required.

The ideal candidate holds an Oklahoma LPN license, with hospice nursing experience preferred and a willingness to participate in on-call rotations. Strong communication and documentation skills are essential.

Qualifications

• Must hold a valid Licensed Practical Nurse (LPN) license in the state of Oklahoma.
• Hospice nursing experience preferred.
• Certification in Hospice and Palliative Nursing desirable but not required.

Responsibilities

• Provide direct nursing care to assigned patients per the physician's plan of care.
• Maintain effective communication with physicians, supervisors, staff, and family members.
• Document nursing observations, interventions, and evaluations promptly.
• Participate in on-call, holiday, and weekend rotations as assigned.
• Assist in evaluating patients for discharge readiness.
• Ensure safe administration of medications and adherence to policies.
