Job summary

Heart to Heart Hospice in Ada, OK is seeking a Licensed Vocational Nurse/Licensed Practical Nurse to join our team. You will assist the RN with the plan of care and support patients and families with compassionate, skilled nursing in home or facility settings.

The role requires an active LVN/LPN license, graduation from an approved nursing program, a valid driver's license, and willingness to provide timely documentation and communicate with the care team.

Qualifications

Active Vocational Nurse License (State) or Practical Nurse License.
Graduate of an accredited school of vocational nursing.

Responsibilities

Assist the RN in carrying out the plan of care.
Supplement nursing care needs as directed by the RN and POC.
Assist the patient in learning appropriate self-care techniques.
Complete clinical and progress notes documenting outcomes of interventions.
Assess patient/family response to care and communicate with the care team.
Timely submission of documentation.
Perform other duties as required.
Ensure compliance with laws, regulations, and policies.
