Job summary

Loving Care In-Home Health Services, LLC is seeking an experienced LPN to coordinate and perform hospice patient visits at Noble Hospice (OK) and Purcell Hospice (OK). You will assess patients, administer medications, and document in the EMR.

The position is full-time with typical hours Monday through Friday, 8:30 am to 4:30 pm, plus on-call rotation for additional pay. A current Oklahoma LPN license and reliable transportation are required.

Qualifications

• Current Licensed Practical Nursing license approved by the Oklahoma Nursing Board without restrictions.
• Preferably one year experience in home health or acute-care setting with geriatric care experience.
• Able to lift 20 lbs. minimum without difficulty.
• Must own a dependable vehicle to do visits.

Responsibilities

• Coordinate working schedule to fit hospice patients into schedule according to patients' time and needs.
• Conduct 30 minutes to 1 hour visits with patients to perform head-to-toe, psycho-social, pain and spiritual assessments, vital signs, administer medications and treatments as ordered by physician.
• Document all activities in the EMR; complete documentation within 24 hours of each visit.
• Notify clients of nursing frequency changes; set weekly/daily schedules to ensure timely visits.
