The Care Team Hospice is seeking a dedicated Licensed Practical Nurse (LPN) to support hospice patients at The Gables of Green in the Akron/Green, Ohio area. This full-time position is primarily facility-based with occasional patient visits within a 20-mile radius, offering meaningful patient relationships and a dedicated hospice team.

• Strong clinical assessment, communication, and patient care skills.
• Active and unrestricted LPN license in Ohio.
• Hospice, home health, skilled nursing, or long-term care experience preferred.

Responsibilities

• Provide compassionate, skilled nursing care focused on comfort and quality of life.
• Monitor conditions, administer meds and treatments as ordered, report changes.
• Build relationships with patients, families, and facility staff.
• Provide education and emotional support to patients and caregivers.
• Complete clinical documentation and collaborate with the interdisciplinary team.
