The Care Team Hospice is seeking a dedicated Licensed Practical Nurse (LPN) to support hospice patients at The Gables of Green in the Akron/Green, Ohio area. This facility-based role focuses on comfort, dignity, and quality of life, with occasional local visits within a ~20-mile radius.

You will work closely with an interdisciplinary team to provide skilled nursing care, education, and emotional support to patients and families, within a compassionate, patient-centered culture.

Qualifikationen

• Active and unrestricted LPN license in Ohio.
• Hospice/home health/long-term care experience preferred.
• Strong clinical assessment, communication, and patient care skills.
• Ability to work independently while collaborating with RNs and physicians.
• Compassionate, dependable, and comfortable providing end-of-life care.
• Valid driver's license, reliable transportation, and current automobile insurance.

• Provide compassionate, skilled nursing care to hospice patients, focusing on comfort and quality of life.
• Monitor patient conditions, administer medications and treatments as ordered, and report changes to the care team.
• Build strong relationships with facility staff, patients, and families for coordinated care.
