Job summary

Day City Hospice in Dayton, OH seeks a Licensed Practical Nurse (LPN) to join a compassionate hospice team. You will provide direct patient care, conduct assessments, and contribute to comfort-focused plans of care in accordance with state standards.

The role offers three 12-hour shifts weekly with every-other weekend, a supportive, team-oriented culture, and opportunities for professional growth including hospice certification.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program.
• Current LPN/LVN license in the state of Ohio.
• Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred).
• Current BLS certification.
• Strong clinical, communication, and organizational skills.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Understanding of hospice philosophy and end-of-life care.
• Experience in hospice or palliative care.
• Familiarity with home-based care and EMR systems.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards.
• Conduct comprehensive nursing assessments and implement individualized plans of care.
• Deliver skilled nursing services focused on comfort, dignity, and quality of life.
• Administer and monitor pain and symptom management interventions.
• Educate patients and families on disease progression, care plans, and symptom control.
• Document patient care accurately and timely within EMR.
• Collaborate with the interdisciplinary team to ensure coordinated, holistic care.
• Evaluate patient responses and adjust care plans as needed.
