Job summary

Qualifications

• High School Diploma or GED required or equivalent.
• Certified Nurse Aide preferred; internal training available.
• CHPNA certification preferred; growth opportunities offered.
• Proficient computer skills to document patient care.
• Valid OH driver’s license with clean driving record.
• Ability to pass background check and 10-panel drug screening.

Responsibilities

• Provide in-home and facility-based hospice care to patients.
• Collaborate with interdisciplinary team to support families.
• Document patient care and services accurately in EMR.
• Travel to patient homes and care facilities within service region.

Skills

Certified Nurse Aide (CNA)

CHPNA certification preferred

Proficient computer skills
