Job summary

Pathways Home Health & Hospice in Sunnyvale is seeking a Hospice Aide/Home Health Aide to provide in-home personal care and supportive services to hospice patients under nurse supervision. The schedule is Monday to Friday with one weekend per month, 8:30 am to 5 pm, serving the San Jose-San Mateo area.

You will work with the case manager to follow a patient care plan, document visits, assist with activities of daily living, maintain safety and infection control, and participate in IDG meetings

Qualifications

• Current California Home Health Aide Certification.
• Interest and ability to work with terminally ill patients; experience preferred.
• Effective communication skills in reading, writing and speaking English.
• Valid California driver's license and auto insurance per agency policy.
• CPR certification; BLS Health Care Provider upon renewal.

Responsibilities

• Provide in-home personal care and supportive services per the patient care plan.
• Assist with activities of daily living and mobility, including transfers and hygiene.
• Communicate with the case manager about patient status and feedback.
• Maintain patient dignity, safety, and infection control in all care.
• Attend IDG meetings and trainings; document visits and schedule changes.
