Job summary

St. Anthony’s Hospice seeks an RN to provide and oversee mission-centered hospice nursing care in patients\' homes or contracted facilities.

You will plan, coordinate, and document care from admission to discharge, coordinating with physicians and the interdisciplinary team per state and federal regulations. The role includes base pay $35-45 per hour plus an estimated $15,000 per year in stipends and bonuses, with duties spanning assessments, pain control, medication administration, and care

Qualifications

• Must hold a current license as a Registered Nurse in Kentucky.
• Minimum of two years of clinical nursing experience, hospice preferred.
• Familiar with computers including internet navigation and email use.
• Must demonstrate commitment to the mission-centered care concept.
• Must meet Federal and State requirements, Medicare Hospice Conditions of Participation, and JCAHO.

Responsibilities

• Perform and document patient assessments per Medicare guidelines and hospice policies.
• Provide hospice-level nursing care per individualized plans of care and regulatory standards.
• Address physical, emotional and social needs of patients and caregivers.
• Conduct weekly visits to assess status and update the Plan of Care.
• Administer medications and monitor pain/symptoms per orders.
• Collaborate with interdisciplinary team and participate in IDT meetings.
• Lead and direct LPNs and HCAs to maximize patient care.
• Complete timely clinical documentation and post-discharge tasks.
