Qualifications

• Active RN license with clinical nursing experience.
• Hospice, palliative, geriatric, or home health experience preferred but not required.
• Ability to remain calm under pressure and support families with empathy.
• Integrity, independence, and a collaborative spirit.

Responsibilities

• Deliver compassionate, skilled nursing care wherever patients call home.
• Collaborate with interdisciplinary team to coordinate patient care and ensure seamless communication.
