Job summary

Elara Caring is seeking a Hospice Aide (CNA) in Illinois to join a dedicated home health team. The role focuses on delivering patient-centered care under the supervision of an RN, with emphasis on safety, comfort, and quality outcomes in the patient’s home.

Qualifications

• HS diploma or GED required.
• 1 year of Hospice Aide or CNA experience is required.
• State-approved Hospice Aide training program that meets regulatory requirements.
• Ability to lift, push, pull, and assist up to 50 pounds.
• Valid driver’s license or reliable transportation to travel to patient homes.
• Home health experience preferred.

Responsibilities

• Provide skilled nursing care per Plan of Care and RN guidance.
• Monitor patient condition and promptly report changes to the RN Case Manager.
• Collaborate with RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques for safety and independence.
• Document medications and maintain accurate medication profiles.
• Help prevent hospitalizations through proactive, coordinated care.
• Communicate compassionately with patients, families, physicians and the care team.
