Job summary

Choice Health At Home is hiring a Hospice Field RN (Registered Nurse) in Ada, OK for full-time home-based care. The role focuses on comprehensive nursing plans, coordination with an interdisciplinary team, and ensuring comfort and dignity for patients and families.

You will conduct initial and ongoing assessments, educate patients and families, and develop plans of care with clinical direction to aides and LPN/LVN. On-call coverage is required.

Qualifications

• Current RN license.
• CPR certification.
• Valid driver's license.
• Valid auto insurance.
• At least 2 years hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' needs related to terminal disease.
• Apply admission and recertification criteria to hospice care.
• Develop and document the plan of care to ensure quality and continuity.
• Educate patients and families about disease process, end-of-life care, nutrition, and self-care.
• Coordinate with IDG members and provide staff training as needed.
• Determine scope and frequency of services based on acuity and caregiver ability.
• Provide clinical directions to Hospice Aide and LPN/LVN.
• Document progress notes and communicate changes to physicians.
• Perform on-call responsibilities as assigned.
