Job summary

John Knox Village Limited is seeking a compassionate Hospice Aide to provide personal care to terminally ill patients under the supervision of the RN Case Manager. You will follow the Plan of Care, document care, and communicate changes to the care team, while upholding Village Hospice standards and safety policies.

Successful candidates will have a high school diploma or GED, Missouri or Kansas CNA certification as applicable, BLS, and a valid driver’s license.

Qualifications

• HS diploma or GED required; combination of education, training and/or experience expected.
• Missouri CNA certification required if working in Missouri; Kansas CNA and Home Health Aide certification required if in Kansas.
• Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
• Must have a valid driver’s license and proof of auto insurance.
• Two years recent CNA experience preferred with at least one in Hospice.
• Certified Hospice and Palliative Nursing Assistant Certification (CHPNA) recommended.
• Ability to read and interpret documents such as safety rules and procedures; strong written/verbal communication.
• Ability to work independently and manage time effectively; establish trust with patients and team.

Responsibilities

• Provides personal care to the patient, assisting with ADLs as directed by the RN Case Manager.
• Follows the Aide Plan of Care and updates RN Case Manager on changes in patient condition.
• Documents care provided and communicates with RN Case Manager and IDT members.
• Completes bedside documentation and ensures visits are locked at completion.
• Synchronizes documentation at the beginning and end of day.
• Performs care aligned with Village Hospice standards of practice and utilizes team members as needed.
• Adheres to safety regulations and maintains professional conduct.
• Attends staff meetings and required in-services.
• Maintains ability to cover routine schedule and respond to emergencies.
