Qualifications

• Must be able to read, write and verbally report clinical information to patients, families, and staff.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Provide direct patient care under RN supervision per Plan of Care.
• Assist with safe transfers and ambulation as required by Hospice policy.
• Document changes and care provided to ensure continuity of care.
• Maintain infection control standards and a clean, safe home environment.
• Communicate respectfully with patients, families, and coworkers.
• Perform other duties as assigned.
