Job summary

New Age Hospice seeks a Certified Nursing Assistant (CNA) or Hospice Aide to provide compassionate in-home care from Cleveland to Livingston, TX. You will assist with daily living activities, ensure a safe environment, and collaborate with the RN and family.

The role requires a High School Diploma (preferred) and completion of a state-approved CNA program or equivalent; hospice/home health experience is encouraged. Reliable transportation and ability to travel locally are essential.

Qualifications

• HS diploma preferred; has completed: nurse aide training or competency evaluation
• Must read, write, and report clinical information clearly
• Completion of Aide Competency Evaluation Skills Checklist and written test

Responsibilities

• Provide direct patient care under RN direction.
• Assist with safe transfers and ambulation per policy.
• Document care and changes to ensure continuity of care.
• Maintain infection control and a clean, safe home environment.
• Communicate respectfully with patient, family, and staff.
• Support nutrition and hydration as prescribed.
