Job summary

Heartland Hospice is seeking a dependable Hospice Aide/CNA to provide compassionate, hands-on care in patients' homes, assisted living facilities, or inpatient units. Join a team dedicated to comfort, dignity, and meaningful moments at end of life.

Responsibilities include direct patient care, hygiene support, mobility assistance, meal prep, emotional support, and careful documentation. A valid CNA license, CPR, driver’s license with reliable vehicle, and at least 6 months of direct patient care

Qualifications

• Active CNA license in the state of employment (Required)
• Current CPR certification (Required)
• Driver’s license and reliable vehicle with insurance (Required)
• Six (6) months of direct patient care experience (Required)
• Comfortable working in home settings, hospice facilities, or long-term care environments
• Demonstrated compassion, patience, and ability to handle emotional situations
• Willingness to work independently and adapt to changing patient needs

Responsibilities

• Provide direct personal care to terminally ill patients
• Assist with personal hygiene, grooming, and toileting
• Support safe mobility using walkers and wheelchairs
• Help with meal preparation, feeding, and light housekeeping
• Observe and report changes in patient condition
• Offer emotional and psychological support to patients and families
• Maintain accurate and timely documentation of visits
• Attend team meetings and participate in Quality Improvement activities
• Practice infection control and follow safety protocols
