Job summary

Heart to Heart Hospice is hiring a Hospice Aide (CNA) to provide direct patient care under RN supervision. You will assist with transfers, document care, and communicate changes to ensure continuity of care.

Requirements include a current CNA license, a high school diploma or GED, and at least one year of relevant experience. A valid driver’s license and reliable transportation are required. We offer comprehensive benefits and growth opportunities.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant in hospital, nursing home, home health, hospice, or LTC facility.
• Demonstrates interest in welfare of ill and elderly; a heart for providing compassionate care.
• Current driver’s license, reliable transportation, and ongoing auto liability insurance.

Responsibilities

• Provide direct patient care under RN direction and according to the Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws and Medicare regulations.
