Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of an RN. The role includes helping patients with transfers, ensuring safety, and documenting care to support continuity of treatment.

Ideal candidates have a current CNA license, at least one year of relevant experience, and a compassionate approach to caring for ill and elderly patients. A valid driver's license and reliable transportation are required.

Qualifications

• Unencumbered CNA certification current on the Nurse Aide Registry.
• Current CNA license.
• At least 1 year experience as Hospice Aide or Nursing Assistant in care settings.
• High School Diploma or GED preferred.
• Interest in welfare of ill and elderly; compassionate care.
• High integrity, including confidentiality obligations.
• EMR experience is a plus.
• Must have a valid driver’s license, reliable transportation, and active auto liability insurance.

Responsibilities

• Provide direct patient care under RN direction and according to the Hospice Aide/Homemaker Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report/document changes and care provided to ensure continuity of care.
• Practice infection control principles.
• Promote positive, respectful communication with patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, regulations, and policies.
