Job summary

Heart to Heart Hospice in Indiana is seeking a compassionate Hospice Aide (CNA) to join our team. You will provide direct patient care under the supervision of the RN and perform routine tasks per the Plan of Care.

Qualified candidates have a CNA license, a high school diploma or GED, at least one year of hospice or nursing assistant experience, and a commitment to dignified care. We offer competitive pay, benefits, and opportunities for growth.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA certification per Nurse Aide Registry.
• Current CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant.
• Interest in welfare of ill and elderly; compassionate heart.
• High integrity and confidentiality.
• EMR experience a plus.
• Must have a valid driver's license and reliable transportation with insurance.

Responsibilities

• Provide direct patient care under RN guidance per Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity.
• Practice infection control principles.
• Maintain respectful communication with patient, family and staff.
• Submit documentation timely.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations and policies.
