Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to care for patients under RN direction. You will assist with transfers, provide essential patient care, and document care to ensure continuity. A compassionate approach and integrity are essential, with EMR experience a plus.

Qualifications

• Must have an unencumbered CNA certification per Nurse Aide Registry.
• At least one year of hospice aide or nursing assistant experience.
• High school diploma or GED preferred.
• Current driver's license and reliable transportation.
• EMR experience is a plus.
• A heart for compassionate care and high integrity.

Responsibilities

• Provide direct patient care under RN supervision and the Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report/communicate changes and document care provided for continuity.
• Practice infection control principles.
• Promote respectful communication with patient, family, and staff.
• Submit documentation timely and accurately.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.
