Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of a registered nurse in Terre Haute, Indiana. You will help with transfers, maintain infection control, and document patient information to support ongoing care.

The role requires an active CNA license, a High School Diploma or GED, and at least one year of relevant experience in hospice or long-term care settings.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA certification/licensure required.
• CNA license current.
• At least 1 year experience as Hospice Aide or Nursing Assistant in healthcare settings.
• Demonstrates compassion for ill and elderly individuals.
• Maintains confidentiality and integrity of information.
• EMR experience is a plus.
• Must have valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist patient with safe transfers and ambulation as per policy.
• Report changes and document care to ensure continuity of care.
• Practice infection control principles.
• Maintain respectful communication with patients, families, and staff.
• Submit documentation promptly and accurately.
• Perform other duties as required.
• Ensure compliance with local, state, and federal regulations and policies.
