Life Care Home Health Family seeks a compassionate Hospice Aide to deliver direct patient care in patients' homes under RN supervision. You will help with transfers, daily activities, and documentation while maintaining infection control standards and patient privacy.

The role requires reading and reporting capabilities, strong interpersonal skills, and willingness to travel locally to patient homes. On-call shifts, weekend and holiday coverage, and ongoing competency development are expected.

Competenze

• Education: High School Diploma preferred; state-approved nurse aide/CNA programs acceptable.
• Experience: 1 year as Hospice Aide, Nursing Assistant, or equivalent recommended.
• Skills: Ability to read, write, and report clinically to patients, families, and staff; strong interpersonal skills.

• Provide direct patient care under RN direction.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.
