Job summary

VitalCaring Group is seeking a compassionate Hospice Aide/CNA to deliver in-home care with dignity and comfort. You will support daily living activities, help maintain safety, and partner with RNs to ensure patient-centered plans are followed.

The role emphasizes empathy, communication, and respect for patient wishes in home settings. You will provide direct hands-on care, document accurately, and collaborate with family members to ease caregiving burdens while aligning with hospice philosophy

Qualifications

Completion of state-required Hospice Aide/CNA training.
Current CPR certification.
Ability to follow care plans, document clearly, and communicate effectively.
Comfort working with end-of-life patients and families.
Strong empathy, compassion, and emotional intelligence.
Reliable transportation and ability to travel between patient homes.

Responsibilities

Assist patients with daily living activities including bathing, dressing, grooming, toileting, and feeding.
Support comfort through repositioning, mobility assistance, and skin integrity care.
Provide light housekeeping to maintain a clean, safe, and comfortable environment.
Offer respite support to caregivers and families.
Observe and report changes in patient condition to the RN/Case Manager.
Document care accurately and timely.
