Custom Care Hospice is seeking a dedicated Hospice Aide to provide compassionate personal care to patients in their Dallas area homes. You will assist with daily living activities, monitor patient well-being, and ensure safety under the guidance of a Case Manager.

The role requires reading and following care plans, CPR certification, and strong communication. Experience in hospice or home health and a calm, empathetic approach are highly valued.

Formación

• Able to read and follow written care instructions and document care.
• Understand hospice philosophy and provide compassionate care to terminally ill patients.
• Satisfactory references from previous employers.
• Self-directed with minimal supervision.
• Comfortable with issues of death/dying.
• Must possess current CPR certification.
• Good oral and written communication skills and strong organizational abilities.

Responsabilidades

• Perform personal care tasks as assigned and respond promptly to patient requests.
• Report any issues to the Case Manager.
• Ensure patient safety and use equipment properly.
• Assist with bathing, oral hygiene, feeding, and changing bed linen.
• Assist with dressing/undressing, meals, and basic nutrition.
• Record vital signs when ordered and as competency is demonstrated.
• Monitor skin integrity and report changes to the Case Manager.
• Assist with ambulation and mobility as directed.
• Provide respite to patient/family/caregiver when on-site as appropriate.
• Maintain clean, orderly living areas and adhere to care procedures.
