Job summary

UnitedHealth Group is seeking a Per diem CNA Hospice Aide to deliver compassionate, hands-on care to patients and support for their families during end-of-life care in Polk County, Florida. The role focuses on daily living activities, comfort, dignity, and emotional well-being under the care plan.

Responsibilities include providing personal care, assisting with mobility and daily tasks, meals, and environment support, while documenting changes for the nursing team.

Qualifications

• Current state CNA certification
• Current CPR certification
• Valid driver's license with reliable transportation

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, ROM, positioning, and homemaking.
• Plans and prepares nutritious meals and assists with feeding.
• Promotes comfort, dignity, and emotional well-being.
• Observes and reports changes in patient status to the nurse.
• Performs tasks per written care instructions from the nurse.
• Assists patients with prescribed exercises.
• Performs simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned
