Job summary

A Path of Care Hospice in Tulsa, OK is seeking a PRN Chaplain to provide compassionate spiritual care to patients and families during challenging times. If you are empathetic, service‑minded, and committed to helping others, this role may be a great fit.

You will conduct spiritual assessments, support bereavement, participate in the interdisciplinary Plan of Care, document patient charts, and build connections with community faith leaders.

Qualifications

• Ordained minister or chaplain in good standing within your denomination.
• Excellent communication skills.
• Ability to work collaboratively as part of a hospice care team.
• Understanding of the grief process for patients and families.
• Reliable transportation and valid Oklahoma driver's license.

Responsibilities

• Conducting spiritual assessments and offering individualized support.
• Assisting with bereavement services as needed.
• Participating in the interdisciplinary Plan of Care to ensure high‑quality spiritual care.
• Maintaining accurate and timely documentation in patient charts.
• Building relationships with community religious and spiritual leaders.
• Coordinating access to community spiritual resources for patients and families.
• Supporting the development of hospice–community partnerships.
