Job summary

Qualifications

• Graduate of an accredited nursing program.

Responsibilities

• Coordinate care with the interdisciplinary team to develop individualized care plans.
• Educate patients and families about hospice services and end-of-life options.
• Document care actions and outcomes in the EMR.
