Job summary

VitalCaring Group is seeking an experienced Clinical Care Manager (RN) – Hospice in Tucson, AZ to lead end-of-life care with compassion, coordinate a cross-disciplinary team, and ensure comfort and dignity for patients and families.

As the hub of care, you will manage plans of care, serve as primary communication between physicians, caregivers, and Intake, and supervise care coordination with an interdisciplinary team.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• 2+ years of leadership, supervisory, or care coordination experience preferred.
• Strong clinical judgment and end-of-life care expertise.
• Excellent communication skills with patients, families, and care teams.

Responsibilities

• Coordinate hospice patient care across the interdisciplinary team.
• Serve as central communication between physicians, caregivers, patients, and internal teams.
• Manage physician orders and ensure timely entry and alignment with the plan of care.
• Partner with Intake to support admissions and follow-up on referrals.
• Lead development and updates of individualized hospice plans of care.
• Support patients and families through education, symptom management, and care planning.
