Job summary

Hospice of the West is seeking a full-time Registered Nurse Case Manager to join our nursing team. You will deliver hands-on hospice care, coordinate services, and support patients and families throughout their hospice journey in the Central and East Tuscon area.

You'll work closely with the Clinical Director, provide compassionate nursing care, and educate families to create personalized care plans. A supportive onboarding and comprehensive benefits package are provided.

Qualifications

• Current unencumbered RN license.
• Minimum one year experience as an RN in medical-surgical/acute care.
• CPR certification as required by state.
• Proficient with electronic medical records (EMR).
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Guide patients and families through the hospice journey with compassion.
• Collaborate with the Clinical Director to assess eligibility for services.
• Provide nursing care rooted in established principles and comfort.
• Educate and empower patients and families to participate in care plans.
• Participate in after-hours call duties as part of the team.
