Responsibilities

• Provide personal care, baths, oral hygiene, and linen changes.
• Assist with meals, feeding, and nutrition planning.
• Monitor and report patient observations to Case Manager.
• Assist in ambulation and range of motion as directed.
• Maintain clean living area and proper documentation.
• Follow safety procedures and avoid nursing tasks.
