Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of an RN. You will assist with transfers, document care, and communicate changes to ensure continuous, compassionate care for patients and families served by our organization.

The role requires an active CNA license, a high school diploma or GED, and at least one year of relevant experience. A valid driver’s license and insurance are required; EMR experience is a plus.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant in a care facility.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.
• EMR experience a plus.
• Must have current driver's license, reliable transportation, and provide ongoing valid auto liability insurance.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report/record changes and care provided for continuity of care.
• Follow infection control principles.
• Communicate positively with patient/family and staff.
• Submit documentation timely.
• Perform other duties as required.
• Ensure compliance with laws and policies.
