Job summary

Heart to Heart Hospice is seeking a Hospice Aide (CNA) to deliver direct patient care under the RN’s guidance and Plans of Care. You will assist with safe transfers and mobility, document care details and report changes to ensure continuity of care.

Infection control, respectful communication with patients and families, and timely documentation are essential. A $500 sign-on bonus and full-time benefits are offered.

Qualifications

• High School Diploma or GED preferred.
• CNA license is current and unencumbered.
• At least 1 year of hospice/nursing assistant experience.
• EMR experience a plus.
• Must have valid driver's license and transportation.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with transfers and ambulation per policy.
• Document changes and care provided for continuity of care.
• Follow infection control principles.
• Communicate with patient/family and staff; timely documentation.
• Other duties as required; comply with laws and policies.
