Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of a registered nurse and within the Plan of Care. You will assist with transfers, report clinical changes, and document care while upholding infection control standards.

Qualifications include a high school diploma or GED, current CNA license, and at least one year of related experience. A valid driver’s license and reliable transportation are also required for patient visits.

Qualifications

• High School Diploma or GED is preferred.
• Current CNA license is required.
• At least one year of hospice aide or nursing assistant experience in healthcare settings.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Follow infection control principles and maintain patient dignity and privacy.
• Communicate effectively with patients, families, and staff.
• Submit all documentation in a timely manner.
• Perform additional duties as required by the agency.
